Family Visions Outreach, Inc. and coordinators of the first Annual FVO Rib Festival will NOT be held responsible for any

lost, due to accidents, theft, bodily injury and, personal injury and including loss of property. Parent/Legal Guardian

consent is required for all minors.

Additionally, I hereby grant full permission to the Family Visions Outreach, Inc. and coordinators and/or agents

authorized by the promoters to use photographs videotapes and other recordings of the 1 st Annual Family Visions

Outreach, Inc. Rib Festival including my name/or voice of any legitimate purpose.
